Please provide any changes in your insurance information as they occur. Inability to provide such information may cause payment issues and/or a disruption in services.
We want to ensure that our team is aware of any changes to your insurance plan, as this can affect payment for your services with your ABA provider.
If you are enrolling in a new plan, we will need a copy of your new insurance ID card to confirm benefits and obtain authorization for the services you are receiving under the new plan.
Please remember that you’re responsible for reporting any changes in your insurance whenever that may occur, either through open enrollment or life events, such as a change in job.
